The Sub-Fascial Breast Augmentation or “Inner Bra” Procedure

While traditional sub-muscular (under the muscle) or sub-glandular (under the breast gland ) implant placements are still used with the right indications  in our practice, commonly seen problems such as implant visibility and implant displacement are often common.  What we refer to as “the Gap” needed a simple and predictable repair, i.e. the replacement of the displaced implants in a new position now known as the “Sub-Fascial Placement”.

The simple fact that more than 50% of our breast surgery practice is dedicated to correcting or revising other breast surgeries helped spark Dr. Drehsen’s pioneering development in the sub-fascial approach, which he has termed the “Inner Bra” technique. This simple procedure has helped him correct numerous implant displacements and repair “cleavage loss” often seen after breast augmentation especially in athletic and physically active women. This correction is achieved by placing new implants under the thin, fibrous layer of the pectoral muscle called the fascia instead of under the moving muscle. In this approach, implants receive strong internal support from the fascia that helps maintain a firm and appealing position of the breast implant.  The Clinique of Plastic Surgery and its medical director, Board Certified Plastic Surgeon, Christian Drehsen, M.D., are well known for this novel technique.

The Sub-Fascial Breast Augmentation Procedure

The surgery usually takes up to two hours and is most often performed under general anesthesia due to the more delicate tissue separation. Upon completion of the surgery, you will typically return home within two hours.  The surgical incisions are similar other breast procedures; however, the axillary approach is the only approach that is not feasible with sub-fascial breast augmentation procedure. 

The Sub-Fascial Augmentation or Inner Bra in Breast Lift Surgery

Our experience with the sub-fascial placement has been extremely favorable in lifting procedures by providing extra support and protection to the exposed breast implants.  We commonly use this approach with the “lollipop” or vertical breast lift (aka “vertical mastopexy”)

How is the “Inner Bra” or Sub-Fascial Breast Augmentation Procedure Helpful?

The inner bra breast procedure differs from traditional cosmetic breast surgeries because it minimizes implant displacement since muscle activity does not affect the implant. It also reduces wrinkling and sagging because the implant is supported by this strong, supporting fibrous sheath.  For women who participate in strenuous exercise, the “inner bra” breast augmentation may be the recommended procedure to avoid implant slippage and maintain more pronounced cleavage.

What you can expect after the Sub-Fascial Breast Augmentation

Recovery time is usually between five to seven days and you can resume normal activities in about three weeks. To reduce the risk of complication, Dr. Drehsen will provide a complete list of post operative care instructions and a surgical bra.

During your recovery, you will experience moderate discomfort, swelling and bruising which is manageable with proper care and medication.  There are also a few restrictions including exercise and lifting objects weighing ten pounds or more. As in any surgery, some complications may occur including infection, bleeding, and excessive scarring.  However these occurrences are infrequent.
